Lewis Middleton was born at Surry Hills and was a van driver. His father lived at Macdonald Street, Manly. He enlisted on 28 February 1916 aged 24.

He embarked on the Warilda on 22 May 1916. He served in France in the 8th Field Company Engineers as a tunneller.

He returned to Australia on the Leicestershire on 3 May 1919.
